In EMT and EMS, SAMPLE is a mnemonic acronym used to remember and systematically collect a patient's important medical history during secondary assessment. It stands for S for Signs and symptoms, what signs you observe and what symptoms the patient reports, such as pain, nausea, dizziness, or difficulty breathing, A for Allergies, asking if the patient has allergies to medications, foods, latex, bee stings, or environmental allergens that could affect treatment, M for Medications, asking what prescription drugs, over-the-counter medicines, vitamins, herbal supplements, or recreational drugs the patient is taking, P for Past medical history, asking about chronic conditions like diabetes, asthma, heart disease, hypertension, seizures, previous surgeries, or similar past episodes, L for Last oral intake, asking when the patient last ate or drank anything, which is important for potential vomiting, aspiration, or need for surgery, and E for Events leading up to the illness or injury, asking what happened, what the patient was doing when symptoms started, mechanism of injury, or any precipitating factors. SAMPLE helps EMTs provide appropriate care and give a thorough handoff to hospital staff.
