A Lutron Maestro switch failing to turn off lights is typically caused by a frozen internal solid-state triac, incorrect load wiring, or a stuck touch button. First, perform a manual power cycle by locating the FASS (Front Accessible Service Switch) air-gap pull-out tab at the bottom of the switch, pulling it out for 5 seconds to break power, and pushing it back in flush.
