How to write a good audit report involves clear structure, objective language, evidence-based findings, and actionable recommendations. Start with an executive summary that briefly states scope, objectives, and overall conclusion for busy readers. Then describe scope and methodology, what was audited, period covered, standards used, and how evidence was collected. Present findings using the 5 C's framework, Criteria what should be, Condition what is, Cause why it happened, Consequence or Effect impact and risk, and Corrective Action or Recommendation what should be done. Use factual, neutral, non-accusatory language, for example instead of saying management failed, say process did not include approval as required by policy. Support each finding with sufficient appropriate evidence and reference documents examined. Prioritize findings by risk and materiality, use headings, bullet points, and tables for readability. Include positive findings where controls work well. Ensure recommendations are specific, measurable, achievable, and include responsible person and timeline. Conclude with overall opinion and follow-up plan. Proofread for accuracy, avoid jargon, and ensure report is concise, objective, and helpful for improvement rather than punitive.
