A toilet tank leaking water continuously into the bowl when no one is using the fixture is caused by an internal tank leak. This is almost always the result of a warped, degraded, or misaligned rubber flapper valve failing to seal the flush valve opening, or a fill valve water level set too high that spills constantly over the top of the overflow tube. Verification: Add food coloring to the tank; if colored water seeps into the bowl within 15 minutes without flushing, the flapper is leaking internally.
