Standard business insurance policies strictly exclude intentional illegal acts, fraudulent behavior, normal wear and tear, and pre-existing damages known prior to policy inception. Furthermore, standard general liability or property packs do not cover employee workplace injuries (which require workers' compensation), professional advice errors (unless professional indemnity is added), cyber-attacks or data breaches (unless specific cyber liability is purchased), or certain catastrophic acts of nature depending on regional exclusions.
